Claims Procedure

Please follow the guidelines below to help Allianz process your claims promptly and efficiently. 

When you visit a medical practitioner, dentist, physician or specialist on an out-patient basis outside the direct settlement network, please take a Claim Form with you and follow the steps below: 

USD Plan

1. You will need to get an invoice from the doctor/medical provider which states the diagnosis or medical condition treated, the nature of the treatment and the fees charged. 

2. Please complete sections 1-5 of the Claim Form. The other sections will only need to be completed, dated and signed by your doctor if the invoice does not state the diagnosis and nature of the treatment (if these details are not shown on the invoice, you are responsible for ensuring that your doctor provides the information requested in sections 6 and 7 of the Claim Form). 

3. When submitting your Claim Form to us, please attach all original supporting documentation, invoices and receipts e.g. medical practitioner/physician invoices and pharmacy receipts with related prescriptions (if available). It is your responsibility to keep copies of all correspondence with us (in particular, copies of Claim Forms and medical receipts). You can submit your Claim Form and supporting documentation to the address indicated on the Claim Form or else via email or fax:

However, we recommend to send to us via [email protected] as member of our Claims Team will personally check your reimbursement. This generally saves you a lot of time and reduces hassle as we would be able to inform you right away if anything is missing. This will also allow us to follow up with Allianz your claims submitted. 

4. An email will automatically be sent to you (where email addresses have been provided to us) to advise you of when the claim has been received and when it has been processed. If we do not hold an email address for you, we will write to you at your correspondence address to advise you when your claim has been processed. 

CNY Plan

If you have treatment on an out-patient basis, you will need to settle the bill directly with the medical provider. You can then seek reimbursement from Allianz China General Insurance Company Ltd. by taking the following steps: 

1. Download the Claim Form (available in English and Chinese) from the downloadable forms section on the right 

2. You will need to get an invoice/FaPiao* from your doctor/medical provider. Please ensure that your invoice/FaPiao states the diagnosis or medical condition that you received treatment for, the nature of the treatment and the fees charged. 

3. Complete sections 1-4 of the Claim Form (the other sections will need to be completed only if the invoice does not state the diagnosis and nature of treatment). 

4. Send the Claim Form and all supporting documentation, invoices and receipts to Allianz China General Insurance Company Ltd. via:

• Fax to: (+ 86) 21 60424688 or

• Scan and email to: [email protected] or

Post to the address shown on the Claim Form (see updated address above) 

However, we recommend to send to us via [email protected] as member of our Claims Team will personally check your reimbursement. This will also allow us to follow up with Allianz your claims submitted.

Claims payment

If you receive treatment in China local regulations state that we can only reimburse your claim in CNY by bank transfer. Please make sure you provide us with your bank account name as it appears on your bank card as well as the name and branch details of your bank.

If you receive treatment overseas and you are a non-Chinese national you can select which currency you would like to receive reimbursement in. However, if you are a Chinese national please note that we are only able to reimburse claims in CNY regardless of where the treatment was received.

FaPiao: a FaPiao is an official receipt that Chinese businesses are required to issue to their customers, upon receipt of payment for a service/product. If you receive treatment in China, your doctor will issue you with a FaPiao instead of an invoice: you will need to submit the original to us in order to seek reimbursement for your medical costs.

**Claims by fax or email: if you receive treatment in China, you should request a FaPiao to your doctor. You can fax or email the FaPiao with your Claim Form – however, the original FaPiao needs to be posted to us in order that we can complete your claim. This applies only to treatment received in China: if you receive treatment outside of China, we do not need the original claim documents to be posted and will process the claim on the scanned or faxed documents only.
